Which I/O do you mean exactly? netPI can be extended with the module called NIOT-E-NPIX-4DIDO https://www.hilscher.com/products/produc...Lang=false. Do you mean this IO?
A word to MQTT itself. MQTT cannot switch on or off any IO. MQTT is just a transport protocol to transport data from a server to a broker and from a broker to a server.
So here is a proposal how I would to it: I would use Node-RED as an instance to control the IO. In Node-RED there is a node to control the IOs of NIOT-E-NPIX-4DIDO and also an MQTT node. Both nodes can be combined together and you can control the IO over MQTT. The Node-RED you need to load on netPI is this one here: https://hub.docker.com/r/hilschernetpi/netpi-nodered
